Transitioning to a New Medical Plan

Transitioning to a different health plan can have its challenges. These how-to’s can help you get the care you need.

Find a Doctor Who Accepts Medicare

You should always choose providers that accept Medicare for Medicare-covered services. Go to medicare.gov to find providers near you that accept Medicare.

Transfer Your Medical Records

If you are selecting a new physician, you’ll need to transfer your medical records from your previous doctor to your new one. Your previous doctor can provide you with instructions.

Transfer Your Prescription Medications

Review the Navitus formulary and prescription drug costs. Talk to your doctor about alternatives to lower your cost.

If you have questions or need help transferring current prescriptions to Navitus, call Navitus customer care at (866) 270-3877, available 24 hours a day, 7 days a week, except Thanksgiving and Christmas Day. TTY users can call 711. We also encourage you to refill any maintenance medications you take so that you have a supply on hand before any change in coverage.

Use Your Member ID Cards

You will and each covered family member will receive two member ID cards—one from Anthem for Benefits Beyond Medicare services (including behavioral health), and a separate card from Navitus for prescription drugs. Present the appropriate member ID card when you visit your doctor or behavioral health professional for the first time or have your next prescription filled.

Each enrolled member will receive their own card with their name on it.

When you receive your new ID cards, review them carefully. Make sure that all of the information is correct. Then replace your old health plan and prescription drug ID cards with the new one.
