Find a Network Physician
The UC PPO plans give you access to one of the largest PPO networks in California. To find a network provider, go to Anthem.
Transfer Your Medical Records
If you are selecting a new physician, you’ll need to transfer your medical records from your previous doctor to your new Anthem Blue Cross network physician. Your previous doctor can provide you with instructions.
Transfer Your Prescription Medications
Review the Anthem drug list and prescription drug costs. Talk to your doctor about alternatives to lower your cost.
We also encourage you to refill any maintenance medications you take so that you have a supply on hand before any change in coverage.

Use Your Member ID Card
You will need to present your member ID card when you visit your doctor or behavioral health professional for the first time or have your next prescription filled.
Your new card will include the subscriber name, as well as the names of dependents covered by the plan.
When you receive your new ID card, review it carefully. Make sure that all of the information is correct. Then replace your old health plan ID card with the new one.
Note that you and each covered family member will receive two separate cards—one for medical and one for pharmacy.
