You pay higher premiums to minimize out-of-pocket costs when you get care from UC Select providers.

Moderate

You pay lower premiums in exchange for higher out-of-pocket costs when you get care.

Low ($0)

You pay no monthly premium in exchange for higher out-of-pocket costs when you get care.

Deductible

Low

No deductible when you see UC Select providers. For non-UC Select providers (Anthem Preferred), you pay the lowest deductible of all the PPO plans.

Moderate

You pay all costs out of pocket (except in-network preventive care) until you meet the plan’s $1,350 (self-coverage) or $2,700 (family coverage) in-network deductible. (Higher deductible for out-of-network care.)

High

You pay all costs out of pocket (except in-network preventive care) until you meet the plan’s $3,000 deductible.

Control over your health care spending

Comfort knowing you have predictable copays for care from UC Select providers and for prescription drugs.

High

Flexibility to control what you pay by using lower-cost in-network Anthem providers, with the option to pay more for out-of-network care.

In-network providers have agreed to charge less for their services. Selecting an in-network doctor will help you avoid surprise expenses when you get care.

Anthem providers outside CA and the U.S.

  • Click on Find a Doctor, Hospital or Urgent Care (under Resources & Tools).
  • Select the plan you are considering (Health Savings Plan, UC Care, CORE)
  • Enter the type of provider you’re looking for, such as primary care or behavioral health.
  • Enter your location and the distance you’re willing to travel.

After you enroll, you can get provider recommendations from Castlight based on where you live and use the online tools to estimate your costs and view quality ratings and patient reviews.
