Find a Network Physician
The UC PPO plans give you access to one of the largest PPO networks in California. To find a network provider, go to anthem.com/ca.
Transfer Your Medical Records
If you are selecting a new physician, you’ll need to transfer your medical records from your previous doctor to your new Anthem Blue Cross network physician. Your previous doctor can provide you with instructions.

We also encourage you to refill any maintenance medications you take so that you have a supply on hand before any change in coverage.
Use Your Member ID Card
You and each enrolled family member will receive an ID card that you'll use for medical and behavioral health services and prescription drugs. Present your member ID card on your first visit to the doctor or behavioral health professional or the first time you have your next prescription filled.
When you receive your new ID card, review it carefully. Make sure that all of the information is correct. Then replace your old health plan ID card with the new one.
