Anthem nurses help people of all ages manage the symptoms of asthma and diabetes. And they work closely with adults who are dealing with chronic obstructive pulmonary disease (COPD), heart failure and coronary artery disease. They can also help if you or a family member is diagnosed with certain types of cancer, vascular or musculoskeletal diseases, or low-back pain. You have toll-free, 24-hour access to nurse coaches, registered nurses who can help you better manage your condition and follow your doctor’s care plan. They can help manage ongoing conditions such as:
- Chronic obstructive pulmonary disease (COPD)
- Heart disease
The ConditionCare team answers questions and provides advice about:
- Eating healthier
- How to get more exercise
- How to quit smoking
- Pain management
